Organization Identification

Indicate the name of the sponsoring organization as it is listed in the University of Chicago Directory, or in the Recognized Student Organization (RSO) database. If the organization is not listed in either, explain the organization’s relationship to, or affiliation with, the University. For RSOs, give the name and contact information of the officer authorized to sign for the organization. Normally this would be the faculty adviser. If particular qualifications, skills or resources of the organization will contribute significantly to the project, please specify these.
